Jazz star guard Donovan Mitchell limped to the locker room with an apparent left hamstring injury late in the Game 5 loss to the Mavericks.
The former No. 13 overall pick was injured on a drive with around 5 minutes left in the fourth quarter and immediately left the game.
He grabbed at the back of his left leg after making a jump pass and didn't make it down the floor to play defense.
The 25-year-old's status for Game 6 on Thursday has not yet been determined, but the signs are ominous at the moment.
The Jazz reported that Mitchell will be evaluated Tuesday in Salt Lake City.
He finished had 9 points on 4-15 shooting, 2 rebounds, 1 assist and 4 turnovers in 31 minutes before exiting.
Mitchell averaged 30.3 points, 4 rebounds and 6 assists in the first four games of the series.
The 102-77 loss moved the playoff series to 3-2 in favor of the Mavericks, with the next game scheduled to take place in Utah.
