Rams' backup center Coleman Shelton suffered an apparent right high-ankle sprain in the first half on Monday night, according to the Pro Football Docs, and was ruled out for the game.
Twenty-seven-year-old rookie Jeremiah Kolone came in to replace Shelton, who started his fourth game in the place of starter Brian Allen.
Allen underwent knee surgery the day after the season opener and was projected to return in 2-to-4 weeks.
Exacerbating the issue is the absence of starting left guard David Edwards with a concussion and right guard Tremayne Anchrum Jr. with a season-ending ankle injury.
Anchrum Jr. started for the first time in Week 2 after Shelton moved over to center and went down on the second play of the game.
The defending champions have rushed for only 217 yards on 3.4 yards per carry through three games for an average of 72.3 yards per game which is the fourth-fewest in the NFL.
San Francisco has sacked Matt Stafford four times through the first half.
