Veteran 49ers' all-pro left tackle Trent Williams limped off the field, eschewing the cart, after suffering a right ankle injury in the third quarter and didn't return.
Based on initial video, the Pro Football Docs have worry Williams suffered a right high-ankle sprain, the same injury that bothered the 320-pound tackle throughout the 2021 season.
Although high ankle sprains are difficult to return from, Williams will likely avoid IR.
Given Williams importance to the offensive line, he may play before he's fully recovered.
Head coach Kyle Shanahan said after the game that Williams “most likely” has a high-ankle sprain, but wouldn't elaborate on the severity or his availability for Week 4.
Williams spent much of the 2021 season on the injury report with ankle and elbow injuries but never missed time due to the ankle sprain and played through the injury against the Packers and Rams in the last year's playoffs.
Williams is the elder-statesman of an otherwise inexperienced San Francisco offensive line that features new faces at center and both guard spots that started a combined three games coming into the season.
Earlier in the third quarter, Jimmy Garoppolo lost a fumble on a botched snap from 30-year-old center Jake Brendel.
San Francisco played much of the second half also without starting linebacker Azeez Al-Shaair.
The fourth-year backer suffered what Kyle Shanahan said was likely a MCL sprain when he took a hit from the helmet of a falling Russell Wilson.
Just like with Williams, Shanahan didn't give any details about the severity of the sprain. Al-Shaair has 11 tackles this season and was leading the 49ers with five tackles before the injury.
San Francisco takes on the defending-champion Rams next Sunday in what multiple players have tabbed as a circled revenge game for last year's NFC Championship.

Azeez Al-Shaair most likely sprained his MCL, and Trent Williams most likely a high ankle sprain, said #49ers coach Kyle Shanahan.
#TrentWilliams leaves with apparent right high ankle sprain. Tough to play thru but he might try.
